Liability Waiver and Media Release
By participating in any activities organized by the Motor City Handball Club (“MCHC”), I acknowledge and agree to the following:
1. Assumption of Risk: I understand that participation in handball and related physical activities involves inherent risks, including but not limited to falls, collisions, injuries, or even death. I voluntarily assume all risks, known and unknown, associated with my participation.
2. Release of Liability: I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless Motor City Handball Club, its organizers, volunteers, sponsors, partners, and facility providers from any and all claims, demands, losses, or liability arising out of or related to any injury, disability, or death I may suffer or loss or damage to person or property that may occur as a result of my participation, whether arising from negligence or otherwise.
3. Fitness to Participate: I affirm that I am physically fit and capable of participating in handball activities. I will not participate if I feel unwell or if advised by a medical professional not to engage in such activities.
4. Emergency Authorization: In the event of an injury or emergency, I authorize MCHC representatives to secure appropriate medical care on my behalf, and I assume full financial responsibility for any medical treatment or transportation.
5. Media Release: I consent to the use of my image, name, and/or likeness in photos, videos, or other media captured during MCHC events for promotional and marketing purposes, without compensation.
6. Code of Conduct: I agree to uphold the values of good sportsmanship, respect, and safety, and will abide by the rules and expectations set forth by MCHC staff and facility partners.
